If the name on the passport does not match what’s on the birth certificate
You must send:
- a signed and dated letter from everyone with parental responsibility confirming the name change and that they agree to the child getting a new passport
- a deed poll
- at least one piece of evidence that shows the new name being used, for example NHS records, child benefits or school records

What documents to send if you apply by post
If you apply by post, you must send:
- 2 new photos of your child
- the child’s full birth or adoption certificate (the one with parent’s details on it)
- proof that your child has British nationality (for example a British registration certificate, parent’s passport details or parent’s birth certificates)
- any valid passports from a different country belonging to the child
- any court orders relating to the child (for example, that describe parental responsibility or residency arrangements)

Choose how you want your documents sent back
Your documents will be sent back to you by normal post, but you can pay an extra £5 to get them sent by secure delivery. Choose this service on your application if you want to use it.
Getting your passport
Your new passport will be sent to you by courier or Royal Mail. They’ll either:
- post it through your letterbox
- hand it to you if you’re home
- leave a card or post you a letter saying how you can get it (it will not say the package is your passport)

You can apply to renew a child passport online or by post.
If your child’s passport has expired, you must renew it before they can travel.
Do not book travel until you have a valid passport - the new passport will not have the same number as the old one.
If your child’s name or other personal details have changed, you cannot renew their passport. You must apply for a new passport instead.
Renewing a passport before it expires
How much time your child needs left on their passport depends on the country they’re visiting. Check the entry requirements of the country they’re travelling to.
Time left on your child’s old passport will not be added to their new one.
If your child’s passport is burgundy or has ‘European Union’ on the cover, they can still use it as long as it has enough time left on it.

Temporary foreign passport for a minor - Poland in Uzbekistan
Where to issue it?An application for a temporary passport for a child can be submitted at any consular office of the Republic of Poland. It should be remembered that you will receive a new temporary foreign passport at the same institution where you submitted an application for its issuance.
Do I need to apply in person?An application for a temporary passport for a child is submitted in person:
- both parents or, respectively, court-appointed guardians,
or - one of the parents or one of the court-appointed guardians,
if they present:
- written consent of the second parent (guardian) to issue a passport to the child, containing a certificate of affixing a handwritten signature by a notary, consul or an employee of the passport office authorized by the voivode,
- a court decision that has entered into force, from which it follows that the second parent (guardian) does not have the right to decide on the issuance of a passport to the child,
- A family court decision that has entered into legal force, in which the court gives consent to the issuance of a passport to a child without the consent of the other parent,
- a complete extract from the Polish birth certificate of the child, from which it follows that the father of the child is unknown (there was no recognition of paternity or establishment of paternity in court),
- an extract from the record of the death of the second parent.
A child who is 5 years old must be present when applying for a temporary foreign passport.

What documents need to be submitted?- A legible application for the issuance of a passport or temporary passport by the consul of the Republic of Poland, which you can obtain at the consulate or download from the website of the consular office.
Parents consent to the issuance of a temporary passport on the application for the issuance of a passport by putting their signatures in the presence of a consular officer, presenting valid passports or identity cards.

- The child's current passport or temporary passport, if such a document has already been issued to him (for presentation).
If one of the parents is applying, please prepare additional one of the following documents:

If, on the basis of the data contained in the documents provided in the case, or the data contained in the registries and lists available to the consul, it is not possible to establish the identity and citizenship of the child, or if these data are in doubt or if discrepancies appear in them, the consul may ask you to provide additional documents of the child, for example:
- abbreviated or full extract from the Polish birth certificate
- Voivode's decision confirming that the child has Polish citizenship
- a valid foreign identity document.
The fee for issuing a temporary foreign passport while waiting for a biometric passport is 15 EUR.
The fee for issuing a temporary travel passport for a purpose other than waiting for the receipt of a biometric passport is 30 EUR.
The fee is collected at the cash desk of the institution at the time of application.

What are the processing times?A temporary passport for a child is issued as soon as possible.
Remember that if you plan to travel to another country on the basis of a temporary passport, you must check the current rules for the entry and stay of foreigners in the territory of this country. You should make sure that the child will be able to cross the border on the basis of a temporary passport and what are the requirements for its validity (for entry and exit).
How to get documents?A child's temporary foreign passport can be obtained personally by one of the parents (guardians) at the institution where the application was submitted.
On the day of receipt of the new document, the child's previous passport (or temporary passport) should be brought to the consulate - the current passport will be canceled (except for pages containing valid visas) and returned.

FAQCan I get a temporary foreign passport?
A temporary foreign passport is issued only in certain cases. Such a passport document can be obtained:
- persons temporarily residing in the Republic of Poland and abroad in order to return to their permanent place of residence,
- persons staying abroad while waiting to receive a passport issued in the Republic of Poland (if they express such a desire and apply for issuance of such a document),
- persons staying in the Republic of Poland and abroad in case of documented emergencies related to the illness or funeral of a relative,
- persons residing in the Republic of Poland and abroad in case of documented emergencies related to their professional activities,
- persons from whom it is physically impossible to take fingerprints, if the absence of such an opportunity is of a temporary nature.
Can I apply for a temporary passport for a child if I have lost his identity card (dowód osobisty) and in Poland a new passport for the child is ready to be received?
You can apply to the Consul for a temporary passport to return to Poland. It should be remembered that when visiting an institution (in Poland) to obtain a new passport, you must have with you a temporary passport issued by the consul.
I lost my child's passport. What should I do?
If you have lost a foreign passport or a temporary foreign passport of a child, you must immediately inform the passport issuing authority - abroad such an authority is the consul.
When a notice of the loss of the passport is submitted to the consulate, the document becomes invalid from the day the consul is notified of the loss of the passport.
Remember! If you find a passport that you have reported lost to the consulate, you should immediately return this document to the passport authority (abroad - to the consul).
